Bespoke Medical Letter | Online Doctor’s Letter | To Whom It May Concern Medical Certificate

Need a medical letter that isn’t covered by one of our standard medical certificates?

Our medical team offers bespoke medical letters for a wide range of personal, educational, employment and administrative purposes. If your request falls outside our standard certificates, one of our GMC-registered UK doctors may be able to provide an individual medical opinion following a clinical assessment.

Each request is reviewed on its own merits. If clinically appropriate, your doctor can issue a personalised medical letter addressed to a named organisation or simply “To Whom It May Concern.”

Our doctors provide independent medical opinions based on:

  • Your completed medical questionnaire.
  • Supporting medical documents or evidence you upload.
  • A telephone consultation where further clarification is required.

Every request is assessed individually. A medical letter will only be issued where the doctor is satisfied it is clinically appropriate and supported by the available information.

What will I receive?

You will receive a verifiable digital PDF letter signed by a medical professional and sent directly to your mobile, containing the following details:

A personalised medical letter written by a GMC-registered UK doctor.

An individual clinical assessment based on your questionnaire and any supporting evidence.

A medical opinion tailored to your specific circumstances.

Confirmation of a medical condition, illness, or fitness/unfitness for a specific purpose, where clinically appropriate.

A letter addressed to a named organisation or “To Whom It May Concern”, if required.

Contact details and QR code enabling a verification check.

As an online service, we are unable to provide GP sickness notes for the UK Home Office, immigration or asylum appointments, court appearances, jury service or workplace tribunals, legal reports, sports medicals, extreme sports, any driving based roles or fitness to fly after a lower limb fracture, recent heart attack or stroke.

What is a Bespoke Medical Letter?

A bespoke medical letter is a personalised document written by a GMC-registered UK doctor when your request is not covered by one of our standard medical certificates. Following a clinical assessment, your doctor will provide an independent medical opinion based on the information you provide, any supporting medical evidence and, where required, a telephone consultation.

Depending on your circumstances, your letter may confirm a medical condition, explain how it affects your daily life, support a return to work or study, recommend reasonable adjustments, or provide an opinion on your fitness or temporary unfitness for a specific activity. Every letter is prepared honestly, accurately and in line with UK medical practice and GMC guidance.

Who Can Benefit from a Bespoke Medical Letter?

Patients request bespoke medical letters for a wide range of personal, educational, employment and administrative purposes. Common examples include returning to work or university after illness, requesting workplace adjustments, confirming a medical condition, or obtaining a "To Whom It May Concern" letter for an organisation.

Our doctors may also consider requests for medical exemptions, such as workplace footwear, shaving requirements or face coverings, where clinically appropriate. Every request is assessed individually, and a letter can only be issued where the available evidence supports the doctor's opinion.

How We Assess Your Request

Your assessment begins with an online medical questionnaire and, where relevant, the upload of supporting medical evidence such as hospital or GP correspondence. Our doctors carefully review all available information and may contact you by telephone if further clarification is needed before making a clinical decision.

Medical letters are only issued where the doctor is satisfied that the request is clinically appropriate and supported by the available evidence.

What We Cannot Provide

For patient safety and medico-legal reasons, we cannot provide every type of medical letter through an online assessment. This includes reports for courts, tribunals, Home Office or immigration matters, as well as medico-legal reports and expert witness documentation.

We are also unable to provide medical certificates for activities that require a face-to-face examination or additional investigations, including scuba diving, skydiving, marathons, triathlons, competitive sports or endurance events.

Fitness to Fly Restrictions

While we provide Fit to Fly certificates where clinically appropriate, there are situations where remote assessment is not considered safe. We cannot issue travel clearance following a recent lower limb fracture, recent heart attack, stroke or transient ischaemic attack, or where significant red flag symptoms or an unstable medical condition are present. These situations require assessment by the treating clinician or another healthcare professional who can perform a face-to-face examination.

Everything you need to know

You can request almost any type of medical letter. Your doctor will decide whether it is clinically appropriate to provide one. Please review our exclusion criteria on our website.

No. Every request is assessed individually. Payment covers the doctor’s clinical assessment. A certificate or letter is only issued if the doctor considers it appropriate.

Not always. Many requests can be assessed using your questionnaire and supporting evidence. However, your doctor may telephone you if further information is required.

Yes. This is one of the most common reasons patients request this service.

No. We do not provide reports for courts, tribunals, Home Office applications or immigration matters through this service.

We only provide full refunds if we are unable to provide a medical certificate.

Once our doctors have provided a medical certificate, we are unable to process a refund.

Our UK doctors at ZoomDoc Health sign all medical letters and certificates.

We are a UK-regulated healthcare service and all our doctors are registered with the UK General Medical Council.
